Medical Release Form

The medical release form is only required if your player has been offered a position on a team.

Instructions for completing the Medical Release Form: 

1. Download and save the Medical Release Form to your computer. (Click here to download the Medical Release Form)
2. Open the Medical Release Form using Acrobat Reader.  Click here to get Acrobat Reader (free) if you do not already have it installed on your computer.
3. Complete and sign the Medical Release Form.
4. Save the file and re-open to verify that your entries were saved.
5. Close the file and use the upload form below to upload and submit your form to us.

2. Complete and sign the Medical Release Form.
3. Scan the completed and signed Medical Release Form as a PDF file.
